Dear Patient,

After considerable deliberation, I have decided to retire from the active practice of medicine effective December 31, 2017. It has been a great honor and privilege to provide care to you and all of my other patients at the Carrollton Eye Clinic over these past fifty-one years. I am proud of the role we have played in this community since we opened our doors in 1976 and while I will miss our patients and our employees, I look forward to the opportunities that lie ahead for me and my family.

Should you choose, you will be able to continue your care with a new practice at the same location, comprised of the same ophthalmologists and medical professionals with whom I have worked at the Carrollton Eye Clinic. I have known these physicians for a number of years and am confident that they and their staff will provide you the same quality care you have received in the past. If you have a future appointment with one of these physicians, you can simply arrive at the same place at the date and time already scheduled. During your first visit, you will be asked to submit a form requesting that your prior two years of medical records be transferred electronically to the new practice.

Should you wish to transfer your records to a different practice, I suggest you contact your health plan and request a list of ophthalmologists in your plan's network. Your written authorization will be required to transfer copies of your medical records to the physician and/or practice of your choice. There are two types of medical records which may apply to your history of care:

Electronic Medical Records ( visits in 2015-2017) - Advanced notice of 10 days is usually required in order to copy your record and fax it. There will be a reasonable fee for this service.

Paper Medical Records (visits any time prior to 2016)-These records are stored off-site at American Document Securities. You may contact them at 770-599-5353 and request a copy of your records. ADS will charge a small fee for this service.
